A check or money order in the amount of $392 (additional $60 if temporary permit is desired) payable to the Minnesota Board of Medical Practice.
One 2"x3” photo on application, notarized next to the picture as a true likeness. The notary seal must fall partly upon the photograph and partly upon the application.(2 licensed physicians must sign off on page.
